for Emergency Hotel/Meal Voucher
Procedure:
A) Case manager shall complete and provide the following
forms to an
available Housing Specialist:
- Program Access Form
- Case
Management Housing Plan Form
- Client Profile Form
- Diagnosis Form (services
will not be provided without a copy of the client's
Diagnosis Form)
- Identification (copy of Driver's License, California
ID, Disabled ID, or
Bus Pass ID)
- Policy and Procedures Agreement
- Disclaimer and Consent to
Release Information Form
B) Obtaining the Emergency Hotel/Meal Voucher from
the Housing
Specialist:
- Housing Specialist is to provide case manager with the actual Emergency
Hotel/Meal Voucher. The Housing Specialist will complete and submit all required documents
on client to the Central Coordinating Agency (CCA).
- The Housing Specialist will disburse, directly to the case
manager, a voucher in a sealed envelope that is stamped along the seal to validate
authenticity.
C) Emergency Hotel/Meal Voucher Extensions:
- Requests for voucher
extensions shall be received from the case manager
at least 24 hours
prior to the expiration of the client's initial voucher.
